The Illinois Department on Aging (IDoA), in partnership with the Illinois Assistive Technology Program (IATP), provides support aimed at enabling social connectedness; maintaining health, safety, and well-being; and promoting independence and community living.

Who is Eligible?

Individuals over 60 connected to, or receiving services from an:

  • Area Agency on Aging (AAA)
  • Community Care Program Care Coordination Unit (CCP-CCU)
  • Adult Day Service (ADS)
  • Adult Protective Services agency (APS)
  • Home Care Ombudsman (HCO)

and are not living in an independent, assisted, or scaled-nursing facility are eligible for the Illinois Care Connections (ICC) program.

*Older adults living in low-income/affordable senior housing/high-rises are eligible to receive services from ICC and should contact any approved aging provider in their area to request a referral.

How do I sign up?

Referrals must be made by an approved service provider. Contact your Area Agency on Aging (AAA) or other IDoA approved provider to find out more or to request a referral for ICC services. You can find your local Area Agency on Aging here:

What Happens Next?

After a referral is received by an approved service provider, every customer of the ICC program completes an intake call with a member of ICC staff to discuss their needs and what devices may best support them.

Please be aware that ICC will call from a 217 area code, and will always leave a message with their name and a callback number when available. Services cannot be rendered if ICC cannot make contact with the referred individual, and the referral will be closed after repeated failed attempts.
